下丘脑结节病一例并文献复习

目的 探讨神经系统结节病的临床特点及治疗方法.方法 对1例经颅脑MRI检查定位下丘脑病变患者,行经纵裂-终板入路部分切除,术后结合激素治疗,病理诊断为结节病,对其病史结合文献进行回顾分析.结果 神经系统结节病临床非常罕见,术前诊断困难,多数确诊神经系统结节病的患者都有全身多系统的侵犯,尤其是肺和心脏.明确病理诊断后,激素治疗效果通常满意.结论 与神经系统症状或体征相对应的临床病史结合病理活检可以明确神经系统结节病的诊断,早期足量应用激素可作为有效的治疗手段.

Hypothalamus neurosarcoidosis:a case report and review of the literature

objective To study the clinical characters and therapy of the neurosarcoidosis.Methods Combined with review of the literatures,the history and clinical data of a patient suffered from hypothalamus neurosarcoidosis,whose diagnosis was confirmed by MRI,corticotherapy and pathological diagnosis after partial resection via interhemispheric trans-lamina terminalis approach,were analyzed.Results The neurosarcoidosis was,very rare.The diagnosis was often difficult before operation.Most patients who suffered neursarcoidosis had findings suggestive of multisystem disease,particularly lung and cardiac involvement.After pathological diagnosis,the effect of corticotherapy was usually satisfactory.Conclusion The diagnosis of neurosarcoidosis is made by observing an appropriate clinical history with neurologic signs or symptoms,accompanied by findings on the biopsy.The early corticotherapy of full dose Was approved to be an effective method.
