腹腔镜下双侧卵巢子宫内膜异位囊肿剔除术90例

目的探讨腹腔镜下双侧卵巢子宫内膜异位囊肿剥除术后卵巢功能早衰、恶变情况及原因分析。方法选取有腹腔镜手术适应证的双侧卵巢子宫内膜异位囊肿患者90例,均采取腹腔镜下囊肿剥除术,分析术中情况及术后随访情况。结果 89例成功实施了双侧囊肿剥除术,1例术中行快速冰冻提示肿瘤有恶性倾向后改开腹手术。患者术后病率(术后24～48 h体温>38℃者)为0,腹部切口甲级愈合率100%;术中出血量平均(56.7±23.4)mL、术后体温恢复正常时间(1.5±0.8)d,排气时间(24.5±9.4)h,术后住院时间(5.1±2.5)d。随访中B超发现卵巢囊肿复发5例,复发率5.6%(5/90);术后痛经症状消失或减轻27例,缓解率87.1%(27/31);14例不孕患者中妊娠5例,妊娠率35.7%(5/14)。1例发生卵巢功能早衰。结论腹腔镜下双侧卵巢巧克力囊肿剥除术是较好治疗方法,但应注意警惕卵巢功能早衰及肿瘤恶变的术中处理。
