摘    要:
目的研究环氧合酶-2(COX-2)蛋白在胃癌、癌前病变、淋巴结转移灶等组织中的表达。方法应用免疫组化SP法和RT—PCR法检测甘肃省河西地区90例胃癌,及其80例增生性病变,22例正常胃黏膜上皮和29例淋巴结转移灶中COX-2的表达。结果免疫组化结果提示正常胃黏膜上皮、增生病变及原位癌中均未发现有COX-2的表达;无淋巴结转移的胃癌原发灶中COX-2的阳性表达率是46%,有淋巴结转移的胃癌原发灶中阳性表达率是72%,淋巴结转移灶中阳性表达率是90%。在鳞状细胞浸润癌中COX-2呈低表达(14%),在浸润性腺癌中高表达(78%)。RT-PCR结果与免疫组化结果相似。结论COX-2的表达与胃癌的浸润、淋巴结转移有关,其表达具有组织选择性。

Abstract:
Objective To investigate cyclooxygenase-2 (COX-2) protein expression in precancerous lesions, primary focus and lymph node metastasis of gastric cancer. Methods 90 gastric carcinoma specimens, 80 gastric pre-malignant lesions, 22 normal gastric mucosa and 29 lymph node metastasis from Hexi district of Gansu province were examined for COX-2 protein expression by immunohistochemistry and RT-PCR. Results In Hexi district, COX-2 protein was not observed in normal gastric mucosa, hyperplasia from mild to severe dysplasia lesions and early-term carcinoma. Positive COX-2 protein expressions were 46% specimens of cancer without metastasis, 72% specimens of cancer with metastasis, and 90% specimens of lymph node metastasis respectively. The expression of COX-2 was lower in invasive squamous carcinoma (14%) than in aggressive adenocarcinoma (78%). The same result was found by RT-PCR. Conclusion The COX-2 protein expression may be associated with the invasiveness and metastasis of gastric cancer and its expression was characterized by the tissue selectivity.
