气管插管和食管气管联合导管应用于院前急救中的对比分析

摘    要:目的探讨在院前急救时喉镜直视下气管插管和食管气管联合导管(ETC)应用价值。方法把2009年1月至2011年1月在院前使用喉镜直视下气管插管和食管气管联合导管(ETC)的患者进行统计分析,将患者分成A、B2组,A组采取喉镜直视下气管插管术建立高级气道,但在气管插管困难时改用食管气管联合导管,B组采取食管气管联合导管。比较两种方法的一次性插管成功率、插管到第一通气的时间及通气效果。结果①A组插管一次成功率为86%,二次成功率为92%,B组插管次成功率达100%,两组插管成功率比较差异有显著性(P<0.05);②A组插管时间为(20±10)s,B组插管时间(40±10)s;A组通气插管成功后通气。结论食管-气管联合导管装置法插管比常规喉镜下气管插管法成功率高,但插管所需时间没有优势,但其一次性成功率高在院前急救可以做为首选。

Abstract:Objective To investigate the practical significance of using ETC orthopticly under laryngoscope outside hospital.Methods the patients who was conducted by ETC orthopticly under laryngoscope outside hospital was analyzed and divided into two groups.In group A,set up airway firstly using tracheal intubation,then chang into ETC;In group B,set up ETC directly.Compare the successful percentage at the first time,the time for ventilation and the effects.Results ①In group A the successful percentage at the first time is 86%,92% at the second time;In group B,the successful percentage at the first time is 100%.The difference between these two groups is significant(P0.05).②It takes(20±10)s for intubation in group A,whereas(40±10)s in group B.Conclusion The successful percentage of ETC is higher than that of intubation under laryngoscope,while the time for ETC is not short.However,ETC can be the first choice during emergency outside hospital due to the high successful percentage.
