脂肪肉瘤上皮性标记物表达的观察分析

目的观察细胞角蛋白（CK）和上皮膜抗原（EMA）在脂肪肉瘤中的表达及特点.方法用常规非生物素过氧化物酶二步法检测14例脂肪肉瘤病例的CK和EMA.结果3例脂肪肉瘤均有上皮性标记的异常表达.阳性信号一般较弱,呈局灶性分布,但也可出现强阳性信号;上皮样细胞或怪异瘤巨细胞EMA表达比CK更强.结论异常上皮性标记免疫反应可以出现在少数脂肪肉瘤.

Aberrant expression of epithelial markers in three liposarcomas with review of literature

Objective Expression of CK and EMA is considered a hallmark of epithelial tumors and thus has been extremely helpful in differentiating carcinoma from other neoplasms of nonepithelial origin such as lymphoma and melanoma,or distinguishing spindle cell or pleomorphic sarcoma from spindle cell(sarcomatoid) or anaplastic carcinoma in the routine practice of surgical pathology.However,the studies have shown aberrant epithelial marker immunoreactivity in a small number of mesenchymal tissues and tumors.Here we showed the evidence of expression of CK and EMA in three liposarcomas and introduced the review of literature.Methods Fourteen cases of liposarcoma were detected with CK and EMA by immunohistochemical method of non-biotin HRP detection system to investigate the expression status of the epithelial markers in the tumor.Results The aberrant immunoreactivity of epithelial markers was observed in three liposarcomas.The positive signal was generally weak and only seen focally,in which expression of EMA seemed to be stronger in epithelioid tumor cells than that of CK.However,strong staining pattern could be also found in some fields. Conclusion Aberrant epithelial marker immunoreactivity can be present in a small number of lipocasarcoma tissues,which needs to be interpreted with caution in clinical application.
