Ⅱ型糖尿病人的胰岛素抵抗与动态血压的关系

分析Ⅱ型糖尿病病人的胰岛素抵抗与动态血压的关系。方法：对74例非胰岛素依赖型糖尿病（NIDDM）病人行24h动态血压监测，其中合并高血压组38人。所有病人行馒头餐试验，测定血糖、胰岛素及血脂，行相关分析。结果：高血压组较血压正常组的胰岛素敏感指数（ISI）显著降低，ISI与24h平均收缩压和舒张压呈显著负相关性，Logistic多元回归分析，ISI与高血压显著负相关（OR＝0．4391P＝0．0311）。结论：提示NIDDM病人合并高血压有更明显的胰岛素抵抗，且胰岛素抵抗在NIDDM病人中仍是高血压的独立危险因子。

RELATIONSHIP BETWEEN INSULIN RESISTANCE AND AMBULATORY BLOOD PRESSURE IN PATIENTS WITH NIDDM

Objective: To analyze the relationship between insulin resistance and ambulatory blood pressure in patients with NIDDM. Methods: 74 subjects with NIDDM wer performed with 24-hour ambulatory blood pressure monitoring, among them 38 patients had hypertension. Results: Subjects with hypertension had significantly lower insulin sensitivity index, compared with 36 subjects without hypertension. Insulin sensitivity indexes (ISI) were inversely significantly correlated with 24-hour mean ambulatory spetolic blood pressure (r = - 0. 255 2, P = 0. 028 2), and diastolic blood pressure (r = -0.467 6, P = 0.000 1). In logistic multiple regression analysis, ISI was independently associated with hypertension (OR = 0. 439 1,95% CI 0. 207 7 ~ 0. 928 1, P = 0. 031 1). Conclusiou: These results suggest that subjects with hypertension are more insulin resistant and hyperinsulinemia than normoensive subjects in NIDDM, and insulin resistance is an independent risk factor for hypertension in patients with NIDDM.
