经皮骶髂螺钉固定技术与开放技术治疗C型骨盆骨折的疗效比较

目的 研究经皮骶髂螺钉固定技术与开放技术治疗C型骨盆骨折的疗效.方法 将C型骨盆骨折患者120例纳入研究,随机分为给予经皮骶髂螺钉固定治疗的观察组和给予切开复位内固定治疗的对照组各60例,观察手术相关指标、远期髋关节功能和活动能力.结果 观察组患者术中出血量（45.8±8.3）ml,切口大小（4.8±0.7）cm,术后下床活动时间（8.2±1.3）d,住院时间（11.5±2.3）d,均明显低于对照组（均P＜0.01）;术后3个月、0.5年以及1年时,观察组患者的Harris评分（64.52±9.31、67.53±10.46、75.87±14.56）及Barthel指数（75.42±9.93、83.48±11.62、89.78±12.64）均明显高于对照组（均P＜0.01）.结论 经皮骶髂螺钉固定治疗C型骨盆骨折有助于减小手术创伤,促进术后恢复,改善关节功能,且取得与常规手术相当的手术时间,是治疗C型骨盆骨折的理想方式.

Effect of percutaneous sacral hip screw and open technology in treating type C pelvic fractures

Objective To study the effect of percutaneous sacral hip screw and open technology in treating type C pelvic fractures.Methods Type C pelvic fractures patients were randomly divided into observation group given percutaneous sacral hip screw and control group given open reduction and internal fixation.Then operation related index,longterm hip joint function and activity capacity were observed.Results The intraoperative bleeding volume,incision size,postoperative ambulation time,hospitalization time of observation group were significantly lower than that of the control group.3,6 months and 1 year after operation and Harris score and Barthel index of observation group were significantly higher than that of the control group.Conclusion Percutaneous sacral hip screw fixation is an ideal way in treating f type C pelvic fractures for,which can reduce operation trauma,promote postoperative recovery,improve joint function,and achieve equal operation time with conventional operation.
