恶性间皮瘤免疫组化诊断30例

目的借助免疫组化技术，在光镜下确诊恶性间皮瘤。方法运用胰蛋白酶消化ＳＰ法及商品化的癌胚抗原（ｃａｒｃｉｎｏｅｍｂｒｙｏｎｉｃａｎｔｉｇｅｎ，ＣＥＡ）、角蛋白（ｋｅｒａｔｉｎ）和波形蛋白（ｖｉｍｅｎｔｉｎ），对３０例恶性间皮瘤组织进行了免疫组化研究。结果（１）恶性间皮瘤组织基本不表达癌胚抗原，藉此可以剔除癌的混杂。（２）恶性间皮瘤具有既表达细胞角蛋白（阳性率４３％），又表达波形蛋白（阳性率７０％）的免疫组化特征，藉此可以与其它间叶来源的恶性肿瘤相区别。结论同时运用３种抗体进行免疫组化染色，可为在光镜下确诊恶性间皮瘤提供可靠的诊断依据。

Immunohistochemical diagnosis of 30 malignant mesothelioma cases

Objective To establish diagnosis of malignant mesothelioma under light microscope with the aid of immunohistochemical staintechnique.Methods Immunohistochemical study in 30 patients with malignant mesothelioma was carried out by using streptavidin peroxidase(SP) method and carcinoembryonic antigen(CEA),keratin and vimentin.Results In general,malignant mesothelioma was negative in carcinoembryonic antigen.Thus we could distinguish malignant mesothelioma from carcinomas.Malignant mesothelioma cells gave positive expression in vimentin and keratin.The positive rate was 70% and 43% respectively.By this observation,we could demonstrate malignant mesothelioma in mesenchymal neoplasm.Conclusion The immunohistochemical stain for carcinoembryonic antigen(CEA),cytokeratin(CK) and vimentin might provide the useful diagnostic basis for malignant mesothelioma under light microscope.
