中重型颅脑损伤患者继发创伤性脑梗死的相关危险因素分析

目的探讨中重型颅脑损伤患者继发创伤性脑梗死(TCI)相关危险因素。方法回顾性分析2008年3月至2010年3月在我院治疗的中重型颅脑损伤患者340例的临床资料,通过对患者的性别、年龄、GCS评分、颅底骨折、脑疝、蛛网膜下腔出血、低血压、糖尿病等因素进行对比以,分析各类因素对继发创伤性脑梗死的影响。结果 340例患者中继发创伤性脑梗死62例,单因素分析结果显示,年龄、GCS、颅底骨折、蛛网膜下腔出血、脑疝、糖尿病和低血压与发生TCI存在相关性,P<0.05;多因素Logistic回归分析结果显示,发生TCI的独立危险因素包括脑疝和低血压,P<0.05。结论 TCI相关危险因素有年龄、GCS、颅底骨折、蛛网膜下腔出血、脑疝、糖尿病和低血压,独立危险因素有脑疝和低血压。

Associated Risk Factors in Severe Traumatic Brain Injury in Patients with Secondary Traumatic Cerebral Infarction

Objective To explore the risk factors related to the traumatic cerebral infarction（TCI） in patients with craniocerebral trauma.Methods A retrospective analysis of data of 340 craniocerebral trauma patients treated in our hospital during March2008 toMarch2010,compared and analyzed gender, age,GCS score,skuU fractures, cerebral hemia, subarachnoid hemorrhage,hypotension,diabetes and other factors influenced on TCI.Results TC1 was found in 62 patients.The results of single factor analysis showed,age,GCS,skull fractures,subaraclmoid hemorrhage,cerebral hernia,diabetes and hypotension were significantly related to TCI,P〈0.05;a multivariate Logistic regression analysis results showed that,independent risk factors of the occurrence of TCI included cerebral hernia and bypotension,P〈0.05.Condusion TCI related risk factors were age,GCS,skull fractures,subarachnoid hemorrhage,cerebral hernia,diabetes and hypotensiun,independent risk factor included cerebral hernia and hypotension.
