绝经前全子宫切除术后对妇女生活质量的影响

目的 :探讨绝经前子宫切除术后对妇女生活质量的影响。方法 :采取问卷调查的形式进行随访 ,以 10 98例子宫切除术后作为研究组 ,95 8例自然绝经者作为对照组 ,研究两组间发生各种症状的发病率 ,经统计学处理有显著性差异 ,P<0 .0 1。绝经前子宫切除组和自然绝经组有膀胱刺激症状者和老年性阴道炎者做膀胱镜检查及阴道涂片 ,对泌尿系统症状者单纯给予尼尔雌醇治疗。对子宫切除组发生精神抑郁症状者 (44 8例 ,占 4 0 .8% )分 A组、B组、C组进行治疗 ,A组用尼尔雌醇 2 mg/ 2 w,B组奋乃静 2 mg 3次 / d,C组尼尔雌醇 2 mg/ 2 w,另加用奋乃静 2 mg 3次 / d,分别于 4 w、 6 w、 8w观察比较。对 3组抑郁症状缓解不明显者转入第 2阶段治疗 ,A组加奋乃静 2 mg 3次 / d,B组尼尔雌醇 2 mg/ 2 w,3组均每晚加安定 5 mg。结果 :研究组发生更年期症状者为 6 1.75 %、性生活障碍者 4 1.99%、精神抑郁症状为 4 0 .8%。对膀胱刺激症状及老年性阴道炎者做膀胱镜及阴道涂片 ,两组无统计学差异 ,P>0 .0 1。研究组发生泌尿系统症状者用药 4 w及 8w,与用药前比较 ,P<0 .0 1;对精神抑郁症状治疗后显效 +有效 ,C组较 A组、B组有比较显著性差异 ,P<0 .0 1。结论 :绝经前子宫切除术后妇女生活质量不仅受更年期症状、性生活障碍、精神

The influence of hysterectomy before menorrhea exhausted on women life quality

Objective: Probe into the influence of hysterectomy before menorrhea exhausted on women life quality. Methods: Follow up with questionnaire investigation form,1 098 cases after hysterectomy as research group, 958 cases natural menorrhea exhausted women as control group, study the incident rate ,there are evidence difference ( P <0.01). The women having bladder stimulated symptom and senile vaginitis in both groups do bladder endoscope and vagina physiopathological determination, the women with urine system symptom are given Niricichun .The psychodumps women are divided into A,B,C groups, A group Niricichun 2 mg/2 w, B group Fentazin 2 mg, 3 times/d, C group Niricichun 2 mg/2 w, adding Fentazin 2 mg 3 times/d, in 4,6,8 w compared respectively, if the revive is not evident, the women will be removed into the second stage treatment ,A adds Fentazin 2 mg, 3 times/d , B group adds Niricichun 2 mg/2 w , the three groups add ANDIN 5 mg, per night. Results: The menopause symptom women are 61.75%, sexual life obstacle is 41.99%, psychodumps is 40.8%. The bladder endoscope and physiopathological determination of two groups has no statistical difference, P >0.01. The urine system symptom women in 4 and 6 w comparing with before drug admonition, P <0.01. The physiodumps treatment effectively in C group comparing with A, B group has signature difference, P <0.01.Conclusion: The life quality of hysterectomy women before menorrhea exhausted is affected by menopause symptom, sexual life obstacle, psychodumps symptom ,and correlates with the deficiency of trigamous hormone .So women before menorrhea exhausted comporated with psychological symptom after hysterectomy should be give antidumps drug go promote women life quality when supplied with trigamous hormone.
