血清肿瘤标志物在腮腺肿物诊断中的价值

目的探讨血清中癌胚抗原(CEA)和糖类抗原(CA)15-3、CA19-9的测定对腮腺肿物临床诊断的价值。方法采用化学发光免疫法,对42例腮腺良性肿物和21例腮腺恶性肿瘤患者血清中CEA、CA15-3、CA19-9的水平进行检测,评价3项指标对腮腺肿物诊断的价值。结果腮腺恶性肿瘤组患者血清中CEA、CA15-3、CA19-9的检测值均高于良性肿物组,但差异无统计意义(P>0.05);恶性肿瘤组中CEA的阳性率高于良性肿物组,差异有显著性(P<0.01);CA15-3、CA19-9的阳性率均明显高于良性肿物组,但差异无显著性(P>0.05)。结论血清中CEA、CA15-3、CA19-9的检测对提高腮腺恶性肿瘤临床诊断率有一定的价值,能在术前确诊近半数的恶性肿瘤,3项指标联合检测可作为腮腺恶性肿瘤诊断的参考依据之一。

Study on detection of tumor markers in serum in diagnosis of parotid tumor

Objective To evaluate the detection of CEA,CA153,CA199 in serum in clinic diagnosis of parotid tumors.Methods The levels of CEA,CA153,CA199 in serum of 42 patients with parotid benign tumors and 21 patients with parotid malignant tumors were measured by chemical luminescence immuoassay.Results The levels of CEA,CA153,CA199 in serum in patients with parotid malignant tumors were higher than those with parotid benign tumors apparently,but the difference between them was not significant(P>0.05).The positive rate of CEA in patients with malignant tumors was higher than those with benign tumors obviously(P<0.01).And the positive rates of CA153 and CA199 in patients with malignant tumors were both higher than those with benign tumors,but the difference between them was not significant(P>0.05).Conclusions The detection of CEA,CA153 and CA199 in serum can improve the accuracy rate of parotid malignant tumors in clinic,and help diagnose about half of malignant tumors accurately before surgery.The combined detection of the three tumor markers formed an important basis for the diagnosis of parotid malignant tumor.
