老年急性胆道感染128例临床分析

目的:探讨提高老年急性胆道感染诊治水平的方法。方法:对128例老年急性胆道感染的临床特点,诊断,治疗与转归等临床资料作回顾性分析。结果:本组128例的主要临床表现有腹痛、腹胀、发热和白细胞计数增高,腹部B超诊断符合率达98.6%(125例)。伴随病较多,其中以心脑血管、呼吸系统疾病及糖尿病多见。非手术治疗84例,手术治疗44例。治愈124例,死亡4例均为非手术治疗者。结论:老年急性胆道感染症状可不典型,诊断应结合临床表现及B超等辅助检查。治疗不论选择手术与否,必须加强对伴随病的治疗,减少并发症以提高治愈率。

Clinical analysis of 128 cases with acute infection of biliary tract in elderly

Objective To improve the diagnosis and therapy of biliary tract in elderly. Method: retrospective analysis of clinical character, diagnosis, therapy and prognosis of 128 patients suffered from acute infection of biliary tract. Results The patients suffered from abdominal pain, abdominal distention and fever. The numeration of leukocyte increased in those patients. Abdominal ultrasonic diagnosis coincidence rate was 98.6% (125 cases). Most of the patients suffered of other diseases, such as cardio and cerebro-vascular diseases, disease of respiratory system and diabetes. 84 cases were treated with non-operative method, and 44 cases were treated with operation. 124 cases were cured, 4 cases were dead who treated with non-operative method. Conclusion The symptom of acute biliary tract infection is not typical. Therefore, abdominal ultrasonic diagnosis and other assisted examination should be taken into consideration when diagnosis. The therapy of associate diseases should be enhanced to decreased the complication and improve the healing rate.
