经腹超声引导绝经妇女取环31例

目的：经腹超声引导下为绝经后妇女取环，增加了取环的成功率，减少出血和并发症。方法：对2002年8月～2007年8月期间，观察31例绝经后妇女，经腹超声引导下取环为实验组（A组），绝经2～7年21例，绝经8～12年10例，绝育器部分嵌入肌层者6例；随机观察门诊绝经两年以上未经超声引导取环者31人，为对照组（B组），绝经2～7年24例，8～12年7例，绝育器部分嵌入肌层者8例。（绝育器不嵌入肌层者在这里不做观察）。观察参数：观察病例数、出血量、取环所用时间，成功病例数，以及成功率。结果：A组31例经腹超声引导下去患者27例成功，4例失败，出血量最多约10ml，取环时间最长40min。另外B组31例，未经腹超声引导取环的患者，其中21例成功，10例失败，出血量最多约15ml，取环时间最长60min。A、B两组相比较，A组成功率与B组成功率相比，差异有统计学意义（P〈0．05）。结论：经腹超声引导下取环，成功率明显提高，出血量明显减少，取环时间缩短，极大减轻了病人的痛苦，这种方法值得推广使用。

Thirty-one cases of removing IUD in postmenopausai women under the guidance of transabdominal ultrasound

Objective： Removing IUD in postmenopausal women under the guidance of transabdominal ultrasound to get a higher success rate and a less secondary complications and hemorrhage. Methods： To observe 31 postmenopausal women from August, 2002 to August, 2007, who were removed IUD under the guidance of transabdominal ultrasound as the experimental group, including 21 cases with menopausal period from 2 to 7 years, 10 cases with menopausal period from 8 to 12 years, and 6 cases with partly trapping into the wall of uterine. 31 stochastic patients who were removed IUD with no ultrasound-guided in the out patient department as control group, including 24 cases with menopausal period from 2 to 7 years, 7 cases with menopausal period from 8 to 12 years, and 8 cases with partly trapping into the wall of uterine. （IUD that has not trapped into the wall of uterine has not been observed.） .To observe parameter, the case number, the blood loss, the time of removing IUD, the number of successful cases, and the success rate. Results： Twenty-seven cases during 31 patients of the A group who were removed IUD under the ultrasound-guided are successful. 4 cases are failed. The most blood loss is about 8 ml. The longest time of removing IUD is 40 minutes. 21 cases during 31 patients of the B group who were removed IUD under no ultrasound-guided are successful. 10 cases are failed. The most blood loss is about 15 ml. The longest time of removing IUD is 60 minutes. To compare the A group with the B group, the difference is significant （P〈O.05）. The difference has statistical significance. Conclusion：The success rate is improved obviously, the blood loss is reduced obviously, the time of removing IUD is shorter obviously, the patient＇s pain is relieved greatly under the guidance of transabdominal ultrasound to remove IUD in postmenopausal women. This method can be used and generalized.
