多层螺旋CT对胆囊癌的诊断及其误诊分析

目的：探讨多层螺旋CT在胆囊癌诊断中的影像特点及其鉴别诊断价值。方法：对21例经手术病理证实的胆囊癌中CT表现及部分病例误诊原因进行回顾性分析。结果：21例胆囊癌中,肿块型11例,其中3例误诊,壁厚型7例,其中2例误诊,结节型3例,其中1例误诊。21例胆囊癌中伴肝内胆管扩张8例,肝脏浸润7例,十二指肠及结肠肝曲受侵犯1例,合并胆囊结石15例,伴肝十二指肠韧带周围淋巴结转移7例。结论：综合分析各型胆囊癌多层螺旋CT表现对胆囊癌正确诊断及其误诊有重要价值。

Diagnostic value of multiple spiral CT on primary gallbladder carcinoma and its misdiagnosis analysis

Objective：To evaluate the value of diagnosis and differential diagnosis for primary gallbladder carcinoma with multiple spiral CT.Methods：21cases with primary gallbladder carcinoma proved by postsurgical pathology were analyzed including the features of CT and causes of misdiagnosis.Results：Among 21 cases of primary gallbladder carcinomas,11 cases（53%） were mass type （3 cases were misdiagnosed）,7 cases（33%） were thick gallbladder wall type and 3 cases（14% ） intracystic nodular type.Of 21 cases of gallbladder carcinomas,8 cases（38%） showed the intrahepatic cholangiectasis,7 cases （33%） were liver infiltration,1 cases（4.7%） was in the infiltration of the duodenum and hepatic flexure of colon,15 cases （71%） were companied with cholecystolithiasis,and 7 （33%） in the metastasis of the peripheral lymph node of duodenohepatic ligament.Conclusion：It is very important to make a diagnosis and reduce the misdiagnosis with the aggregate analysis for different primary gallbladder carcinomas by multiple spiral CT scan.
