三种手术方式治疗腰椎间盘突出症的临床疗效比较

目的：评价MAsTQuadrant通道下手术治疗腰椎间盘突出症临床疗效。方法：随机抽取66倒患者分成3组，其中22例在Quadrant通道下行腰椎间盘突出症髓核摘除术，22例行椎间盘镜下髓核摘除术，另外22例患者采用传统开窗手术，随访10-18个月（平均12．3个月），并比较术前、术后2周以及术后6个月VAS和JOA评分。结果：3组均能有效缓解患者腰腿痛等临床症状；在手术切口、术中出血量、住院时间以及术后疼痛等方面Quadrant组和MED纽要优于传统开窗组（P〈0．05），但Quadrant组和MED组之间比较差异无统计学意义（P〉0．05）。结论：Quadrant通道下手术是治疗腰椎间盘突出症的一种“直视下的微创”手术方法，具有创伤小、出血少、恢复快、住院时间短、术后疼痛轻等优点。

Comparison of the Clinical Efficacy of Three Different Surgical Treatment of Lumbar Disc Herniation

Objective:To evaluate the curative effect of surgical treatment on lumbar disc herniation by using MAST Quadrant system.Methods:All 66 patients were randomly divided into three groups,and the three groups were received surgical treatment by using MAST Quadrant system,micro-endoscopic discectomy(MED),and traditional fenestration respectively.They were followed up for 10-18 months(12.3 months averagely),and the VAS and JOA scores were compared preoperatively and postoperatively.Results:The three groups can effectively alleviate low back pain and other clinical symptoms of the patients.There was no significant difference between the Quadrant group and MED group in length of incision,blood loss,hospital stay and postoperative pain(p>0.05),but they were superior to the traditional fenestration group(P<0.05).Conclusion:Surgical treatment on lumbar disc herniation by using MAST Quadrant system is a "orthophoria minimally invasive" method with advantages of fewer traumas,less bleeding,faster recovery,shorter hospital stay,and less postoperative pain.
