糖化白蛋白与糖尿病肾功能的关系研究

摘    要:
目的:探讨血清糖基化白蛋白(GA)水平与2型糖尿病肾功能不全的关系及其检测意义。方法:选正常人群98例为正常对照纽(A纽);另选2型糖尿病患者153例,根据其血清肌酐清除率(Ccr)分为4组:B组(12例)Ccr≤30mL·min^-1;C组(62例)30mL·min^-1〈Ccr≤60mL·min^-1;D组(61例)60rnL·min^-1〈Ccr≤90mL·min^-1;E组(18例),Ccr〉90mL·min^-1测各组血清GA水平及糖化血红蛋白水平(HbAlc)。结果:GA水平B组(19.32%±4.26%)、C组(17.03%±3.41%)、D组(16.37%±4.00%)、E组(16.87%±4.27oA)与A组的GA水平(14.98%±1.64%)均有显著性差异。C组、D组、E组的GA水平与B组的GA水平有显著性差异。HbA1c水平B组、C组及D组与A组有显著性差异,E组与A组差异无显著性。B组、C组、D组、E组之间的HbA1c均无显著性差异。结论:2型糖尿病患者合并严重肾功能不全时GA水平升高,它可能是2型糖尿病合并肾功能不全的危险因素。

Abstract:
Objective: to analyze the relationship between glycated albumin and renal function among diabetics, and find out the correlation between the both. Methods: We choose 98 normal persons as control group (group A)and 153 diabetics patients. According to the Ccr , the diabetics patients were divided into four groups, as group B (Ccr≤30 mL · min^-1 ,n = 12) , group C (30mL · min^-1Ccr≤60 mL · min^-1, n =62) , group D(60 mLmL · min^-1Ccr≤90 mL · min^-1, n =61)and groupE (Cer〉90 mL · min^-1, n = 18). Results:statistical analysis results displayed the GA was significant difference among group A (14.98% ± 1.64%),group B(19.32% ± 4.26%),group C(17.03% ± 3.41 %) ,group D(16.37% ±4. 00V0)and group E(16. 87% ±4.27%) but the HbAlc was significant difference among group A, group B ,group C and group D,instead there was no difference with group E. Conclusion: HbA1c and GA are both indicators for blood glucose control in diabetics. Better than HbA1c , GA indicated the renal function degreely in diabetics.
