选择性应用人工股骨头置换术治疗高龄股骨粗隆间骨折

摘    要:目的探讨选择性应用人工股骨头置换术治疗高龄合并骨质疏松股骨粗隆间骨折的临床疗效。方法用骨水泥型双极人工股骨头置换治疗年龄在75岁以上的股骨粗隆间骨折24例。结果大部病例术后2~3w可下地行走,关节活动度基本正常。结论选择性采用骨水泥双极人工股骨头置换术治疗高龄合并骨质疏松股骨粗隆间骨折,手术操作相对简单、模式化、时间短,手术创伤相对较小,术后可避免因闭合或切开复位内固定带来的诸多并发症和长时间的骨愈合期,能早期进行关节活动和负重训练,缩短患者卧床时间等优点,是内固定治疗很好的补充。结合改善骨质疏松治疗,有效提高了生存质量。

Experience of selective application of artificial prosthesis replacement therapy for the treatment of intertrochanteric fractures in elderly patients

Abstract:Objective To explore the clinical efficacy of selective application of artificial prosthesis replacement therapy for the treatment of osteoporotic intertrochanteric fractures in elderly patients.Methods Twenty-four patients over 75 years old with osteoporotic intertrochanteric fractures were treated with bone cement-type artificial femoral head prosthesis replacement.Results Most patients could ambulate in 2-3 weeks after surgery with nearly normal range of motion.Conclusion Selective application of artificial prosthesis replacement therapy for the treatment of osteoporotic intertrochanteric fractures in elderly patients possesses many advantages,for example,relative simple,modular,short procedure with little trauma,less complications induced by close or open reduction and internal fixation,short bone healing period,performing joint motion and weight training at early stage,and short lying-in-bed time.This is great supplement for the internal fixation therapy.This therapy combined with the treatment to ameliorate bone mass can effectively improve the life quantity.
