全盆腔脏器切除治疗晚期直肠癌15例分析

目的 探讨全盆腔脏器切除治疗晚期直肠癌的临床疗效.方法 采用Miles手术方式为基础,包括肿瘤在内全部切除直肠、乙状结肠远侧段、膀胱、远侧输尿管、后尿道、肛门及盆腔内的生殖器官,同时行盆腔内的侧方淋巴结清扫,并进行随访.结果 本组无手术死亡,术后恢复较顺利,5年生存率达45%左右,生活质量均有不同程度的改善.结论 晚期直肠癌广泛邻近器官浸润,行全盆腔脏器切除,可获较满意的姑息疗效.

Analysis of 15 patients with advanced rectal carcinoma carried by TPE

Objective To explore the therapeutic effect of total pelvic exenteration(TPE) for advanced rectal carcinoma.Methods 15 patients with advanced rectal carcinoma were operated with TPE,including total rectal,distal end of sigmoid colon,bladder,posterior urethra,anus,intrapelvic sexual organ and side lymph node,and the survival rate was estimated.Results No perioperative death occurred,5 year survival rates were 45%,and survival quality was improved to different extent.Conclusion Content therapeutic effect was acquired by TPE to advanced rectal carcinoma extending to adjacent organs.
