18 F-FDG PET和PET-CT显像在肺癌诊断与分期中的临床应用价值

摘    要:目的:评价18F-氟代脱氧葡萄糖(FDG)正电子发射计算机断层摄影术(PET)及18F-FDG PET显像与CT扫描融合显像(PET-CT)对肺癌诊断与分期的临床应用价值.方法:对45例经病理证实为肺癌和10例良性病变的18F-FDG PET和PET-CT显像资料进行回顾性分析,分别分析PET和PET-CT影像,对病灶进行诊断和定位.结果:PET和PET-CT诊断肺癌的灵敏度为100%,特异性分别为70%和90%;PET诊断肺癌肺内转移、纵隔转移、脑转移的灵敏度分别为91%、93.3%、50%,特异性分别为95.5%、96%、100%;PET-CT显像诊断肺癌肺内转移、纵隔转移、脑转移的灵敏度分别为100%、100%、100%,特异性分别95.5%、96%、100%.PET和PET-CT在诊断肺癌其它远处转移病灶时亦具有很好的准确性.结论:PET、PET-CT对肺癌诊断与分期均具有重要的临床价值,是一种有效的无创性诊断技术;PET-CT比PET有更精确的立体空间分辨率,在精确定位、病灶形态学观察和发现一些小的隐蔽性病灶方面更具优势.

Abstract:Objective:To assess the value of clinical application imaging with 18F-FDG PET only and PET-CT fusion in lung cancer diagnosis and staging.Methods: 18F-FDG PET and PET-CT imaging materials of 45 cases with lung cancer and 10 cases with benign lesions were reviewed.All the cases were imaged before surgical operation or biopsy.The accurance of diagnosis in lung cancer and location of lesions with PET only and PET-CT fusion were analyzed and compared.Results:The sensitivity and specificity of PET only and PET-CT fusion for detection of malignant mass were 100%,70% and 100%,90%,respectively.The sensitivity and specificity of PET only were 91%,93.3%,50% and 95.5%,96%,100% in diagnosis of metastasis lesions in lung,mediastinum and brain,while that of 100%,100%,100% and 95.5%,96%,100% with PET-CT fusion.PET or PET-CT imaging was also very accurant in remote metastasis of lung cancer.Conclustion: 18F-FDG PET and PET-CT imaging are powerful and uninjury diagnostic methods in lung cancer diagnosis and staging.It has an obvious advantage in spatial resolution,lesion locating and morphology study and hidden smaller lesions revealing with PET-CT fusion comparing with PET only.
