2型糖尿病患者早期肾脏损伤检测指标探讨

目的：探讨2型糖尿病人早期肾脏损伤检测指标。方法：根据UAE将83例2型糖尿病人分成3组，分别对这3组以及正常健康受试者测定了24小时尿微量白蛋白、尿NAG。结果：微量白蛋白尿组同正常对照组比较，其mALB和NAG升高，其差异具有统计学意义（P〈0．01），大量白蛋白尿组同正常对照组比较，其mALB和NAG升高明显，具有显著差异性（P〈0．001）。正常白蛋白尿组的mALB和NAG同正常组比较有所升高，但是其差异不具有统计学意义（P〉0．05）。同时将mALB和NAG进行相关性分析，二者具有正线性相关关系（P〈0．05），说明，在mALB升高的同时，NAG也随之升高，甚至，在一些患者中，NAG可以先于mALB发生改变。结论：联合mALB和NAG检测，对2型糖尿病患者肾脏损伤的早期诊断具有一定的临床意义。

Study of Indicators for Diagnosing Type 2 Diabetic Nephropathy in Early Stage

Objectives:To explore the indicators for diagnosing type 2 diabetic nephropathy in early stage. Methods:83 patients with 2 type Diabetes were divided into 3 groups according to the UAE. The levels of mALB and NAG the patients and the control group were measured. Results:Compared to the control group, the levels of NAG and mALB were significantly raised (P<0.01) in group B and C. While there were differences between group A and the control group, but the difference wasn't significant (P>0.05). At the same time, the relationship was analyzed between mALB and NAG. They showed good positive correlation (P<0.05), which indicated that NAG would increase as mALB was increasing. Even some patients showed that NAG was changing before mALB was changing. Conclusions:Combination of mALB and NAG measurement would be helpful to diagnosis of diabetic nephropathy in its early stage.
