血栓性血小板减少性紫癜19例临床分析

目的：研究血栓性血小板减少性紫癜（ＴＴＰ）的临床诊断及治疗方法。方法：对１９例ＴＴＰ患者进行回顾性分析。结果：１９例均表现有微血管病性深血性贫血、血小板减少、精神神经异常等三联征，同时有肾损害、发热等五联征者１３例。血浆置换或血浆输注有效率为７５．０％（９／１２），单用激素和护因小板药物治疗有效率为１４．３％（１／７），两组显著性差异（Ｐ〈０．０５）。结论：临床遇急性溶血伴精神神经异常并有出血倾向

Clinical analysis on 19 cases of thrombotic thrombocytopenic purpura

Objective: To study the clinical diagnosis and treatment of thrombotic thrombocytopenic purpure (TIP) . Methods: Analyzed 19 cases of TFP retrospectively. Results: 19 cases (100%) had clinical manifestations of micmangiopathic hemolytic anemia, thrombocytopenia and neurological dysfunctiton, 13 cas- es (68.4%) had the pentad of the above manifestations, impaired renal function and fever. The response rate to plasma exchange (FE) and plasma infusion (PI) was 75.0% (9/12), and that of glucocorticoids and antiplatelet therapy was 14.3%, (1/7) . Difference between the two groups was significant (P <0 .05) .Canclusion:It should be considered of UP for patients with acute hemolysis, neurological dysfunction and haemonhagic tendency. PE and PI were considered to die optimal first - line treatment for TTP.
