超声胃镜检查对胃癌术前分期的诊断价值

目的探讨超声胃镜（EUS）检查对胃癌术前分期的诊断价值。方法对45例胃癌患者进行术前超声胃镜检查和术后组织病理检查，行TNM分期，将超声胃镜检查结果与术后病理检查结果进行比较。结果与术后病理结果对比，EUS的T分期总准确率为88．9％，T1到T4各期准确率分别为75．0％、87．5％、90．4％、91．7％。EUS的N分期总准确率为75．5％，N0、N1、N2分期的准确率分别为77．4％、72．7％、66．7％。结论EUS能够比较准确地判断胃癌的浸润深度及周围淋巴结转移情况，对胃癌的TNM分期、可切除性及预后的判断有较大的临床价值。

Diagnostic Value of Echogatstroscope on Preoperative Staging of Gastric Cancer

Objective To study the diagnostic value eehogatstroscope on preoperative staging of gastric cancer. Methods TNM stage of 45 gastric cancer patients were examined with preoperative echagatstroscope and postoperative histopathological examination, the results of echogatstroseope were compared with result of histopathological examination. Results Compared with definite pathological, T staging of echogatstroscope classification were correct in 88.9% of all cases, and the accuracy of T1 , T2, T3 and T4 were 75.0% ,87.5% ,90.4% and 91.7%. The accuracy of N staging was 75.5% ,and the accuracy of No ,N1 and N2 were 77.4% ,72.7% and 66.7%. Conclusion EUS has high accuracy to diagnose the invasive depth of gastric cancer and lymphatic metastasis. It has great clinical value on the preoperative TNM staging of gastric cancer, the judgement of resectability and the prognosis.
