创伤评分系统在严重创伤患者伤情评估中的应用初探

目的 研究创伤评分系统对严重创伤患者伤情评估的效果.方法 选择2000年1月～2008年12月以腹部损伤为主并进行剖腹手术救治的192例严重创伤患者的临床资料,其中男性146例,女性46例;年龄7～65岁,平均年龄35.1岁.交通事故伤77例,高处坠落伤36例,刀刺伤及其他伤79例.通过创伤评分系统进行创伤评分.根据救治的最终结局,将患者分为存活组和死亡组两组,统计分析并比较小同创伤评分指标的差异.结果 两组患者在年龄、呼吸指标差异无统计学意义,但在其他临床观察指标及创伤评分指标差异均有统计学意义(P＜0.05).结论 创伤评分系统是对创伤患者的损伤严重程度和结局进行评估的有效方法,值得进一步推广应用.

Application of trauma scoring system in injury evaluation of severe trauma

Objective To investigate the effects of trauma scoring system on injury evaluation in severe trauma patients. Methods A retrospective review of trauma database from January 2000 to December 2008 was conducted, wherein 192 abdominal surgery with severe trauma patients were enrolled, of which 146 were male and 46 were female, aged 7 - 65 years old（mean age 35.1 years old）. Seventy-seven of them suffered from traffic accident, 36 were injured by falling from high places, and 79 were stabbed or injured by other causes. A comparison of demographics, injury severity scores （ISS）, revised trauma scores （RTS）, and trauma score-injury severity scores（TRISS） were conducted between survival group and death group. Statistical analysis was performed using SPSS 13.0 statistic software package. Results No significant differences were found in terms of age and respiratory rate between the survival group and death group. All other clinical parameters and trauma scores were significantly different between the 2 groups（P 〈 0.05）. Conclusion It is demonstrated that the trauma scoring system is valuable in injury severity evaluation and outcome prediction of severe trauma patients, which is worthy of further application.
