鼻咽通气道在肥胖早孕者行无痛人工流产中维持气道的作用

目的 探讨肥胖早孕者行无痛人工流产术中,应用鼻咽通气道维持上呼吸道通畅的临床妓果.方法 选择门诊无痫人工流产中肥胖的早孕者40例（BMI≥30）,随机分为2组（n=20）：A组（鼻咽通气道组）和B组（对照组）.均采用丙泊酚与舒芬太尼静脉复合全身麻醉.A组在睫毛反射消失后放置鼻咽通气道,B组未放置.全程观察并记录RR、SpO2、上呼吸道梗阻情况.结果 两组术中RR差异无统计学意义（P〉0.05）;两组术中出现90%〈SpO2〈95%及SpO2〈90%的例数,A组均明显少于B组（P〈0.05）;两组上呼吸道梗阻情况对比A组较B组轻（P〈0.05）.结论 肥胖早孕者行无痛人工流产中,应用鼻咽通气道可以维持上呼吸道的通畅.

Application of nasopharyngeal airway in the obese patients undergoing painless induced abortion

Objective To evaluate the effectiveness of nasopharyngeal airway on relieving airway obstrucion in the obese women with early pregnancy undergoing painless induced abortion. Method Forty early pregnant women （BMI≥30） in out-patient clinic were randomly divided into 2 groups （n=20）, group A with nasopharyngeal airway and group B without （control group）. All subjects accepted total intravenous anesthesia with sufentanil combined with propofol. Nasoparyngeal airway were placed on after lash reflex was disappeared in group A, but not in group B. Results There were no statistical differences in both two groups in RR. Patients in group A with SpO2 between 90%-95% or below to 90% were less than that in group B（P〈0.05）. Compared with the group B, the upper airway obstruction situation was slighter in group A. Conclusion The application of nasopharyngeal airway on painless induced abortion can obviously relieves the airway obstruction in obese women with early pregnancy undergoing painless induced abortion.
