复方苦参注射液在晚期恶性腹腔积液患者姑息治疗中的疗效分析

目的观察复方苦参注射液对晚期合并恶性腹腔积液肿瘤患者的生活质量的影响。方法选择50例晚期肿瘤合并恶性腹腔积液患者,根据治疗方法不同分为2组：观察组（复方苦参加支持疗法）、对照组（支持治疗）,观察组在支持治疗基础上加用复方苦参注射液腹腔灌注,每次100 mL,24 h重复l次,2周为1疗程,2疗程后评价2组患者腹水缓解率、KPS评分、NRS评分变化及外周血淋巴细胞免疫表型（CD3＋、CD4＋、CD8＋及CD4＋/CD8＋）的变化。结果与对照组比较,观察组在腹水缓解率、KPS评分、NRS评分方面差异有统计学意义（P〈0.01）,外周血CD3＋、CD4＋、NK、CD4＋/CD8＋比值增高,CD8＋比例降低,与对照组比较差异有统计学意义（P〈0.01）。结论复方苦参注射液腹腔局部灌注在晚期恶性腹水肿瘤患者的姑息治疗上,可激活患者的免疫功能,改善生存质量。

Analysis of the efficacy of compound matrine injection in palliative treatment of patients with advanced malignant ascites

Objective To observe the effect of compound matrine injection on life qualities of patients with advanced cancer and malignant ascites. Methods According to different treatment methods, 50 patients with advanced cancer and malignant ascites were divided into two groups： observation group and control group. Patients in observation group were treated by compound matrine injection （peritoneal perfusion, 100 mL, q24 h, for 2 weeks） and supportive care. Patients in control group were treated by supportive care only. After 2 treatment cycles, the remission rate of ascites in patients, KPS score,NRS score and phenotype of peripheral blood lymphocyte（CD3 ＋ , CD4＋ , CD8＋ and CD4＋/CD8＋ ）were detected and recorded and the data were statistically analyzed. Results In terms of remission rate of ascites in patients, KPS score, NRS score, the difference in observation group were statistically significant compared with the control group （P 〈 0.01 ）. Values of CD3＋, CD4＋ , CD8 ＋ and CD4＋/CD8 ＋ increased and the value of CD8 ＋ reduced. There was significant difference compared with the control group （ P 〈 0.01 ）. Conclusions Peritoneal perfusion with compound matrine injection is suitable for patients with advanced malignant ascites in palliative treatment because it can activate the patient＇s immune function and improve their life qualities.
