| 摘 要： | 目的：研究胸椎黄韧带骨化症（OLF）的诊断与手术治疗方法,探讨其病因、病理、手术效果与并发症。方法：采用回顾性研究方法对手术治疗的46例OLF病例进行分析。手术方法均采用椎管后壁切除术。结果：术后40例随访1.5年以上,6例经短期观察。优20例,良17例,可5例,差4例。结论：OLF所致的脊髓病表现复杂,手术治疗必须慎重操作。手术近期效果良好。以“孤立-磨薄-分割-切除”的方法行椎管后壁切除减压是治疗本病安全、有效的方法。

| Abstract: | Objective: To study characteristics of diagnosis and surgical treatment of ossification of ligamentum flavum (OLF) in the thoracic spine,and investigate its etiology,pathology,operative effect and complications.Methods:Forty six cases of OLF treated surgically from 1989 to 2000 were studied retrospectively.Results:40 cases had been followed up for 1.5 year ,6 cases had been observed for a short time.Excellent in 20 cases,fine in 17 cases,fair in 5 cases,poor in 4 cases.Conclusion:The Clinical features of thoracic myelopathy caused by OLF were very complex.Operative management must be careful.Operative intervention had favorable effect on the patients with OLF.Removing the posterior wall of the thoracic spine canal with the technique of "isolating thining separating removing" is a safe,convenient and effective method for the surgical treatment to the patients with OLF. |
