2型糖尿病合并高血压患者血清网膜素、脂联素及内脂素水平变化

摘    要:
目的探讨2型糖尿病(T2DM)合并高血压患者血清网膜素、脂联素及内脂素水平的变化。方法 102例T2DM患者,根据是否合并高血压分为T2DM合并高血压组(n=50)和T2DM无高血压组(n=52)。检测2组患者的空腹血糖(FBG)、糖化血红蛋白(Hb A1c)、空腹胰岛素(FINS)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平;并检测糖耐量试验(OGTT)2 h后的血糖(2h BG)、胰岛素(2h INS)水平及空腹和OGTT 2 h后网膜素、脂联素、内脂素水平。结果 2组患者血脂、FBG、FINS、2h BG、2h INS水平及胰岛素抵抗指数(HOMA-IR)比较,差异无统计学意义(P>0.05)。2组患者空腹网膜素、脂联素及内脂素水平与OGTT后2 h比较,差异无统计学意义(P>0.05)。T2DM合并高血压组患者空腹及OGTT后2 h网膜素和脂联素水平低于T2DM无高血压组(P<0.05),内脂素水平高于T2DM无高血压组(P<0.05)。血清内脂素水平与HOMA-IR和FINS呈正相关(P<0.05);血清网膜素和脂联素水平与HOMA-IR和FINS呈负相关(P<0.05)。结论 T2DM合并高血压患者血清网膜素、脂联素及内脂素水平明显改变,且与胰岛素抵抗有一定的相关性。

Abstract:
Objective To investigate the changes of serum retinal, adiponectin,visfatin levels in diabetes mellitus combined with hypertension patients. Methods One hundred and two patients with type 2 diabetes mellitus(T2DM) were divided into T2DM with hypertension group( n = 50)and T2DM without hypertension group (n = 52)according to whether combining with hypertension. The levels of fasting blood glucose ( FBG ), glycosylated hemoglobin ( HbA1c ), fasting insulin ( FINS ), total cholesterol ( TC ), triacylglycerol ( TG), low density lipoprotein cholesterin ( LDL-C ), high density lipoprotein cholesterin ( HDL-C ) were detected, and two hours after the oral glucose tolerance test (OGTr), the 2hBG,2hINS levels were detected. The retinal, adiponectin and visfatin levels of fasting and two hours after OGTF were detected too. Results There was no significant difference in the level of blood fat, FBG, FINS, 2hBG, 2hINS and HOMA-IR between the two groups ( P 〉 0.05 ). Two hours after OGTY,there was no significant difference in the level of retinal, adiponectin and visfatin between the two groups( P 〉 0. 05 ). The levels of retinal and adiponectin of fasting and two hours after OGTr in T2DM with hypertension group were lower than those in T2DM without hypertension group (P 〈 0.05 ), while the level of visfatin was higher than that in T2DM without hypertension group (P 〈 0.05 ). There were positive correlations between the level of visfatin and HOMA-IR, and FINS (P 〈 0. 05 ). The levels of retinal and adiponectin were negative correlated with HOMA-IR and FINS( P 〈 0.05 ). Conclusion The levels of retinal, adiponectin and visfatin in T2DM combined with hypertension patients have changed significantly, and there is certain correlation with insulin resistance.
