血必净注射液对脓毒症大鼠白细胞介素-6表达及血小板的影响

摘    要:
目的观察中药血必净注射液对盲肠结扎穿孔术(CLP)所致脓毒症大鼠血浆白细胞介素-6(IL-6)、外周血单个核细胞IL-6mRNA表达和血小板计数(PLT)的影响。方法将78只清洁级Wistar大鼠按随机数字表分为正常组、假手术组、CLP组和血必净组,后3组按活杀时间再分为手术后12h、24h、48h和72h4个亚组,每组6只。各组于相应时间点经腹主动脉取血,采用酶联免疫吸附法(ELISA)检测血浆IL-6水平,分离单个核细胞应用实时荧光定量聚合酶链反应检测IL-6mRNA的表达,并观察大鼠PLT的变化。结果CLP组大鼠血浆IL-6水平以及外周血单个核细胞IL-6mRNA的表达在术后各时间点均显著高于正常组(P〈0.05或P〈0.01),而PLT明显低于正常组(P〈0.05或P〈0.01)。血必净注射液干预后,血浆IL-6水平以及外周血单个核细胞IL-6mRNA表达显著低于CLP组(P〈0.05或P〈0.01),PLT在干预后24h、48h、72h也恢复至正常范围。结论血必净注射液能显著降低脓毒症大鼠血浆前炎症因子IL-6蛋白质以及mRNA表达水平,并能明显升高PLT,从而防止脓毒症的进一步发展。

Abstract:
Objective To investigate the effects of injection of Xuebijing, a preparation of traditional Chinese drugs, on the plasma interleukin-6 (IL-6) levels and IL-6 mRNA expression in the peripheral blood mononuclear cells (PBMCs)and platelet counts (PLT) in sepsis. Methods Forty-eight male Wistar rats underwent cecal ligation and puncture (CLP) to cause septic rat models and then randomly divided into 2 equal groups: Xuebijing-treated group, injected intravenously with Xuebijing 4ml/kg 2, 12, 24, 36, 48, and 60 h after CLP, and CLP group injected intravenously with normal saline 2,12, 24, 36, 48, and 60 h after CLP. Twenty-four Wistar rats underwent sham-operation and 6 rats were used as normal controls. 12, 24, 48, and 72 hours after the operation blood samples were obtained from the abdominal aorta. Enzyme-linked immunoadsorbent assay (ELISA) was used to determine the levels of plasma IL-6 levels, and IL-6 mRNA expression in the PBMCs was examined by RT-PCR. The changes in PLT count were also observed. Results Compared with the normal control group, the plasma IL-6 levels and IL-6 mRNA expression level in the PBMCs at different time points were significantly higher (P〈0.05 or P〈0.01), and the PLT was significantly decreased (P〈0.05 or P〈0.01) in the CLP group. However, in the Xuebijing-treated group the plasma IL-6 levels and IL-6 mRNA expression levels at different time points were all significantly decreased (P〈0.05 or P〈0.01), and the PLT count peaked 24h later and returned to the baseline value 24-72 h after Xuebijing treatment. Conclusion Xuebijing injection significantly down-regulates the plasma IL-6 levels and IL-6 mRNA expression, and increase the PLT level, thus preventing the development of severe sepsis secondary to acute insults.
