彩色多普勒血流成像在卵巢囊性血肿鉴别诊断中的应用

目的 探讨彩色多普勒血流成像(CDFI)在卵巢囊性血肿鉴别诊断中的应用价值.方法 采用CDFI分别对21例卵巢囊性血肿患者和72例卵巢其他囊性肿物患者进行检查,观察病变的边界、囊壁及内部分隔带的厚度、内部回声,并运用CDFI检查肿物边缘及内部的血流强度,以病变周边检出环状或半环状血流作为诊断卵巢囊性血肿标准,应用频谱多普勒测量病变边缘和内部血流的RI和PI值.超声诊断结果与手术病理结果对照.结果 CDFI显示21例囊性血肿边界均清晰、内部分隔带纤细且杂乱,其中15例囊壁周边见丰富的环状或半环状彩色血流信号,CDFI诊断符合率为90.5%(19/21),CDFI检查卵巢囊性血肿的敏感性为71.4%,特异性为100%.72例卵巢其他囊性肿物中,良性肿物61例,恶性肿物11例.11例恶性卵巢囊性肿物边缘及内部见丰富的彩色血流信号,其边缘血流与卵巢囊性血肿边缘血流比较,RI值及PI值差异无统计学意义(P＞0.05).结论 CDFI对卵巢囊性血肿的诊断率高,有重要的临床应用价值.

Color Doppler flow imaging for differential diagnosis in ovarian cystic haematoma

Objective To probe the value of color Doppler flow imaging(CDFI) in differential diagnosis of ovarian cystic haematoma. Methods Twenty-one ovarian cystic haematoma and 72 other ovarian cystic lesions were examined with CDFI. The boundary and interior echoes, wall and interior compartments were observed. The blood flow in periphery and interior of the lesions was detected with CDFI and the values of RI and PI were measured with pulsed Doppler. Ring-like or semiring-like flow in the periphery was considered as the standard for diagnosing ovarian haematoma The results of CDFI were compared with those of pathology after operation. Results Clear boundary and fine and clutter interior compartments were observed in 21 ovarian cystic haematomas. CDFI showed rich circularity or half-circularity blood flow in 15 cases of 21 ovarian cystic haematomas. The diagnostic rate, sensitivity and specificity of CDFI in ovarian cystic haematoma were 90.5%(19/21),71.4% and 100%, respectively. Seventy-two other ovarian cystic lesions included 61 benign lesions and 11 malignant cystic lesions. Rich blood flow signal was observed in periphery and interior of 11 malignant cystic lesions. The RI and PI of periphery blood flow in ovarian cystic haematoma were not different from those of ovarian malignant cystic lesions (P>0.05).Conclusions CDFI is of high diagnostic rate in ovarian cystic haematoma and of important clinical application value.
