临床药师参与1例脑梗死伴血小板减少患者的药物治疗实践

目的:探讨临床药师对脑梗死患者实施药学监护的意义。方法:对1例脑梗死伴血小板减少患者进行全程药学监护,从抗血小板治疗、他汀类药的应用、脑保护剂的应用等方面分析患者的药物使用情况。结果:临床药师针对患者具体情况,协助医师制订治疗方案,治疗18d后患者面部及肢体感觉障碍消失、病情平稳,治疗过程中无出血事件发生,后康复出院。结论:临床药师通过参与临床实践,可协助医师提高药物治疗的安全性和有效性,使患者得到优良的药学服务。

Experience of Clinical Pharmacists Participating in Treatment for a Cerebral Infarction Patients with Thrombocytopenia

OBJECTIVE：To discuss the significance of clinical pharmacists providing pharmaceutical care for cerebral infarction patients.METHODS：Clinical pharmacist provided whole-course pharmaceutical care for a cerebral infarction patient with thrombocytopenia,and drug use of the patient was analyzed in respect of antiplatelet treatment,statins use and brain protecting agent.RESULTS：Clinical pharmacists assisted physician to develop therapeutic scheme according to specific information of patients.After 18 day of treatment,facial and limb sensory disturbance of patient had disappeared with stable condition.No hemorrhage event occurred during treatment.The patient was discharged from hospital after healing.CONCLUSION：Through participating in clinical practice,assisting physicians to improve safety and efficacy of drug treatment,clinical pharmacists provide patients good pharmaceutical care.
