退行性腰椎滑脱伴椎管狭窄症的治疗方法探讨

1985年以来,32例退行性腰椎滑脱伴椎管狭窄症通过随机选择,分为两个手术治疗组。所有病人均得到随访,其中16例施行单纯椎板减压术,优良率仅43.7％;16例施行开窗减压+后外侧融合术,优良率达93.8％。我们认为,开窗减压+后外侧融合术具有手术创伤小,术后腰椎稳定性好,硬膜及神经根不易发生疤痕粘连。因此,术后在稳定腰椎、缓解腰腿痛方面均明显优于单纯的椎板减压术(P<0.001)。

Research of the Treatment of Degenerative Lumbar Spondylolithesis with Spinal Stenosis

Since 1985, thirty-two consecutive patients were assigned alternately to two groups of treatment: One group of decompressive laminectomy (sixteen patients) and the other of decompressive fenestration and posterolateral -spinal fusion (sixteen patients). All the patients were followed up. The result of the first group excellent and good were 43.7%, while the other group were 93.8%. The treatment of the second group had much advantages: yidlding less trauma to lumbar spinal; better postoperative stability; less scar tissue adhering to the dura and the nerve root. Therefore, the result of this operation was signifcantly better than that of simple decompressive laminectomy with re-spest to stability of lumbar spine and relief of pain in the back and lower limbs (P<0.001).
