妊娠期糖尿病的筛查及治疗效果观察

目的 :研究妊娠期糖尿病 (GDM)的发生率 ,对血糖异常的孕妇行饮食治疗 ,从而降低母婴的并发症。方法 :对 1860例孕妇进行葡萄糖筛查试验 ,将妊娠期血糖异常者 ,分为单纯 5 0 g阳性、GIGT、 GDM三组。观察各组治疗与未治疗二者之间母婴并发症的分布情况。结果 :血糖异常 195例 ,发生率为 10 .48%。其中 5 0 g阳性 12 9例 ,占 7.17% ;GIGT3 5例 ,占 1.94% ;GDM3 1例 ,占 1.72 %。饮食治疗总有效率为 90 .18% ,其中 5 0 g阳性为 97.2 2 % ,GIGT为 82 .76% ,GDM为 69.2 3 %。治疗组母婴并发症较未治疗组差异有显著性 (P<0 .0 5 )。结论 :妊娠期血糖异常的发生率为 10 .48% ;饮食疗法是治疗妊娠期血糖异常的主要方法 ,治疗有效者母婴并发症的发生率明显下降

SCREENING AND THERAPY OF GESTATIONAL DIABETES MELLITUS

Objective:To investigate the occurrence rate of GDM and reduce the maternal-fetal complication of pregnant women with abnormal blood glucose level through dietary treatment Methods:The blood glucose level of 1860 pregnant women was examined and those with abnormal blood glucose level were divided into three groups: group 50g(+), group GIGT, group GDM. Maternal-fetal complications were observed among each group with or without treatment.Results:195 pregnant women were found blood glucose level abnormal with an occurrence rate 10 48%. Among them, 129 women were found only with positive reaction 50g(+), accounting for 7 17%; 35 with GIGT, accounting for 1 94% and 31 with GDM, accounting for 1 72%. 90 18% of all the patients was assessed as effective through dietary treatments, 97 22% in group 50g(+); 82 76% in group GIGT; 69 23% in group GDM. The maternal-fetal complications were significantly deceased in the group with treatment( P <0 05) in comparison with the group without treatment. Conclusions:The occurrence rate of gestational abnormal blood glucose level is 10 48%. Dietary treatment is the main therapy method and the maternal-fetal complications can be significantly decreased in well-controlled patients.
