经皮椎体成形术治疗椎体隐匿性骨折的临床疗效观察

摘    要:
目的 观察经皮椎体成形术(PVP)治疗骨质疏松所致椎体隐匿性骨折的临床疗效,讨论其应用价值.方法 因老年骨质疏松引起的椎体隐匿性骨折患者25例(共35个椎体),采用椎体成形术治疗,术前、术后3d及术后6个月应用视觉模拟评分法(VAS)对疼痛程度进行评估分级,并观察伤椎椎体形态学变化.结果 25例患者术后3d及6个月后疼痛程度明显减轻,与术前比较差异有统计学意义(P<0.01),术后3d、6个月后椎体高度相对于术前无明显变化,差异无统计学意义(P>0.05),有1个胸椎体的椎间隙渗漏,无脊髓压迫症状,也无肺栓塞、感染等并发症 结论 经皮穿刺椎体成形术治疗老年骨质疏松性椎体隐匿性骨折可迅速缓解疼痛;并可显著预防椎体进一步塌陷,防止脊椎后凸畸形发生.

Treatment of Subtle Fracture of Osteoporotic Vertebral Body by Percutaneous Vertebroplasty

Abstract:
Objective To study the results of percutaneous vertebroplasty ( PVP ) in treating subtle fracture of osteoporotic vertebral body, and to explore the clinical value of PVP in patients with subtle fracture of osteoporotie vertebral body. Methods Twenty-five patients (35 osteoporotic vertebral body )with subtle fracture of osteoporotic vertebral body were collected. They were all treated by PVP. The al- leviation of the pain and the heights of the vertebrae were measured at 3 days and 6 months after therapy. Results The pain of all the patients relieved significantly at 3 days and 6 month after operations. The differences had satistical significance( P 〈 0.01 ). The heights of the veterbral bodys showed nearly difference at 6 months of follow. The two differences had not satistical significance ( P 〉 0.05 ). Conclusion PVP is an efective procedure for relieving the pain of the patients with subtle fracture of osteoporotic vertebral body and could effectively prevent collapse of veterbral body.
