微创穿刺加尿激酶灌注引流治疗高龄高血压脑出血

目的探讨高龄高血压脑出血的微创治疗。方法将49例高龄高血压脑出血患者随机分为微创组20例,对照组29例。微创组行血肿腔穿刺＋尿激酶灌注溶解引流术治疗,对照组行开颅血肿清除治疗。统计治愈率、显效率、并发症发生率。结果微创组死亡3例,对照组死亡9例,2组比较差异有统计学意义（P〈0.05）。微创组显效8例,并发症8例;对照组显效8例,并发症29例。两组病例随访半年,微创组治愈16例,对照组治愈17例。2组对比分别行χ2检验,均P〈0.05,有统计学意义。结论微创穿刺加尿激酶灌注引流术是治疗高龄高血压脑内出血的一种有效方法。

Direct cleaning by drilling combined with lysis and drainage of hematoma with urokinase in aged patients with HIH

Objective: To explore the effects of mini-traumatic operation in treatment of HIH in aged patients.Methods:Forty-nine aged patients with HIH were randomly divided into 2 groups,with 20 in the mini-traumatic operation group(treatment group),and 29 in control group.The treatment group were treated with drilling combined with lysis with urokinase and drainage,and the contronl group received the craniotomy to evacuate the hematomas.All the data,including cure rate,effective rate and incidence rate of complications,were collected and the results were dealt with statistics SPSS software package.Results:Three were dead in the treatment group,and 9 in the control group.There was significant difference between the two groups(P<0.05);the results showed 8 effective cases and 8 with complications in the treatment group,and 8 effective cases and 29 cases with complications in the treatment group.The two groups were followed up by half a year,and the cured cases were 16 in the treatment group and 17 in the control group.The result from the chi-square criterion showed the cure rate in treatment group was significantly different from that in the control group(P<0.05).Conclusion: Direct cleaning by drilling combined with lysis and drainage of hematoma with urokinase is an effective and minimally invasive method for aged patients with HIH.
