脂肪肝患者肝静脉多普勒频谱变化研究

摘    要:目的:探讨肝静脉频谱在不同程度的脂肪肝患者中的变化情况及对脂肪肝的诊断价值。方法:对93例脂肪肝患者及50例健康者进行了肝静脉彩色多普勒检查,并将脂肪肝分级结果与肝静脉频谱变化进行相关性比较。结果:按肝静脉频谱特征将其分为3类:①0型(HV0):三相波,存在短暂反向血流;②1型(HV1):双相波,频谱波动振幅减弱,无反向血流存在;③2型(HV2):单相波,频谱波动消失,波形平稳。98%的健康对照组和51.61%的脂肪肝患者为0型。2%健康对照组和46.24%脂肪肝患者为1型。健康对照组未见2型,脂肪肝患者中2型为2.15%。脂肪肝的分级和肝静脉的频谱变化之间存在着直线相关关系(P<0.05)。健康对照组与肝病组之间肝静脉频谱异常有显著性差异(P<0.01)。结论:肝静脉多普勒检查有助于脂肪肝的诊断及分级。

Doppler spectrum changes of hepatic vein in patients with fatty liver

Abstract:Objective: To evaluate the relationship between the Doppler spectrum of hepatic veins and fatty liver. Methods: Color Doppler examinations of hepatic veins were performed in 93 cases of patients with fatty liver and 50 healthy subjects as the control. The correlative comparisons were done between Doppler spectrum changes of hepatic veins and different grades of fatty liver. Results: The patterns of Doppler spectrum were classified into three types according to the spectrum characteristics: ①type0 (HV0), triphasic waveform with a short phase of reversed flow; ②type1(HV1), biphasic waveform with amplitude of the phasic oscillations decreased- and with no short phase of reversed flow; ③type2 (HV2), uniphasic waveform with no phasic oscillation- and the waveform being flat. Type 0 waveform was found in 98% of the healthy subjects and 51.61% of the patients with fatty liver; Type 1 waveform was found in 2% of the healthy subiects and 46.24% of the patients with fatty liver; Type 2 waveform wasn't found in the healthy subiects and 2.15% of the patient group. There were linear correlations(P<0.05) between the severity of fatty liver disease and changes of Doppler waveform of hepatic vein. There were significant differences (P<0.01) in Doppler waveforms of hepatic vein between the control group and the patients classified by the severity of fatty liver respectively. Conclusion: Doppler spectrum of hepatic veins may be useful for the diagnosis evaluation of the fatty liver disease.
