多烯酸乙酯和阿托伐他汀对高脂血症患者血脂和CD40影响的比较

摘    要:
目的:比较多烯酸乙酯和阿托伐他汀对高脂血症患者血脂和CD40水平的影响.方法:62例原发性高脂血症患者,男44例,女18例,随机分成两组,多烯酸乙酯组(F组)30例,阿托伐他汀组(A组)32例,分别应用多烯酸乙酯和阿托伐他汀治疗8周,测定治疗前(O周)、治疗4周和8周的血脂、CD40的水平.结果:治疗8周后两组甘油三酯(TG)、载脂蛋白B(apoB)均降低(p〈0.05),总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)阿托伐他汀组显著下降(p〈0.01).两组治疗前较正常人群CD40表达明显上调,而治疗后CD40均有下降,但仅多烯酸乙酯组有统计学意义(p〈0.05);结论:高脂血症刺激细胞CD40表达显著,多烯酸乙酯除具有降脂作用外,还具有抑制CD40-CD40L信号转导通路的作用,这可能是其抗炎、抗动脉硬化的重要机制之一.

Abstract:
Objective: To compare the effects of ethyl polyenoate and atorvastatin on plasma lipid levels and CD40 expression of patients with hyperlipidemia. Methods: Sixty two patients with primary hyperlipidemia (44 male and 18 female) were randomly divided into two groups and treated with ethyl polyenoate (Group F, 30 cases) and atorvastatin (Group A, 32 cases), respectively, for 8 weeks. The plasma lipid levels and CD40 expression levels were measured before treatment started (Week 0), and 4 weeks and 8 weeks after treatment. Results: After 8-week treatment, TG and apoB were remarkably decreased in both groups (p〈0.05) compared with baseline. TC and LDL-C were notably reduced only in Group A (p〈0.01). Compared with healthy control, CD40 expression was significantly upregnlated in two groups before treatment (p〈 0.05).The decrement of the expression was observed in both groups, but only that of Group F had statistical significance (p〈0.05). Conclusion: Hyperlipidemia stimulates the upregnlation of CD40 expression significantly. Ethyl polyenoate is capable to not only lower plasma lipid level but also inhibit the CD40-CD40L pathway, which may contribute to its anti-inflammatory and anti-atherosclerogenic property.
