腹腔灌注榄香烯注射液联合微波热疗治疗癌性腹水的临床研究

目的：观察腹腔灌注榄香烯注射液联合微波热疗治疗癌性腹水的有效性及安全性。方法：将72例癌性腹水患者随机分入观察组（36例）和对照组（36例）,腹腔穿刺放尽腹水,观察组于腹腔内注入榄香烯注射液后进行微波热疗,每周2次,连续2周;对照组单纯腹腔内注入榄香烯注射液。4周以后,评价两种方法对癌性腹水的有效性以及安全性。结果：治疗后观察组的有效率为77.8%,优于对照组的50.0%,差异具有显著性;KPS评分及体重的稳定率方面,观察组均优于对照组,差异具有显著性;生存期方面,观察组的中位生存期和3个月、6个月、1年的生存率均优于对照组,其中6个月、1年的生存率上二者存在显著性差异。治疗后两组均未出现骨髓抑制、肝肾功能损害,仅少部分出现恶心呕吐、发热、腹痛等不良反应,两组间差异无统计学意义。结论：腹腔灌注榄香烯注射液联合微波热疗可以控制癌性腹水,改善患者的生活质量,延长生存期,且不良反应轻微,对于癌性腹水的治疗具有一定的价值。

Intraperitoneal Infusion of Elemene Injection Combined with Microwave Hyperthermia in Treatment of Patients with Malignant Ascites

Objective： To study the efficacy and safety of intraperitoneal infusion of Elemene Injection plus microwave hyperthermia for patients with malignant ascites. Methods： Seventy- two patients with malignant ascites were divided into observation group and control group randomly. All patients were performed abdominocentesis and ascites was drained and then patients of observation group received perfusion of Elemene Injection plus microwave hyperthermia twice a week,four times totally. The other group received intraperitoneal perfusion of Elemene Injection only. And the evaluations of effect and toxicity were performed after four weeks. Results： After treatment,the response rate of the observation group was77. 8%,better than the that of control group,showing the significant difference. The stable rate of KPS＇score and body weight about the observation group were superior to those of the control group,showing the significant difference. The median survival time and the three months,six months,one year survival rate were better than those of the control group,a-mong which,the six months and one year survival rate showed the significant difference. No bone marrow suppression,liver or renal impairment occurred after treatment in both groups. Nausea,vomit,fever,abdominal pain and other adverse reactions appeared in few patients of the two groups,showing insignificant difference. Conclusion： Intraperitoneal infusion of Elemene Injection plus microwave hyperthermia has a good effect in both the treatment of malignant ascites and the improvement of the quality of life with slightly adverse reactions.
