免疫印迹试验带型与HIV-1感染关系的探讨

目的了解免疫印迹试验(WB)带型与HIV-1感染的关系,探寻WB确证问题处理办法。方法收集2004-2009年上半年经首次确证为HIV-1抗体阳性或不确定结果并进行了追踪复检的检测对象,对其复检结果和流行病学资料进行分析。结果初检WB带型为p17、p24p17、p66p17者复检均转阴,为p24、gp160、gp160gp120、gp120p24、gp160p24者有阳性或阴性两种转归;初检包含两条env带或包含一条env带和p24带的3条或3条以上条带者复检结果均为阳性。复检阴转者主要来自于无偿献血,自愿咨询检测(VCT)、孕产妇、术前检查、婚/体检。复检间隔时间为2 d至11个月,其中阳转间隔最短5 d,最长6个月,平均为51 d;阴转间隔最短2 d,最长11个月,平均为129 d。另1例艾滋病病人初检带型为gp160gp120;1例阴性转归者带型为gp160p66p51p24。结论建议WB出现"2条env带或1条env带+p24带"者判为不确定,在此基础上至少增加1条带(无论env、gag或pol带)才判HIV-1抗体阳性;对WB不确定带型或可疑带型结果的感染判定必须结合带型特点、流行病学资料和临床特征综合分析,并可通过缩短随访时间达到尽快确定感染状况的目的。
