儿童后尿道瓣膜造影诊断及球囊治疗

目的 采用逆行性膀胱造影和排泄尿道造影,显示后尿道瓣膜及并发的尿路梗阻征象。从而做出影像学诊断。将球囊导管插入后尿道扩张、疏通后尿道膜性梗阻。方法 应用以上方法诊断后尿道瓣膜１６例,患儿年龄３个月～４岁,平均年龄２岁６个月。本组病例中３例经造影诊断后行外科手术治疗,１３例采用后尿道球囊治疗。１６例患儿均有出生后排尿困难、尿线不连贯、滴沥状临床特点。结果 １２例造影中直接显示后尿道瓣膜负影,全部病例

Posterior urethra valves in children: diagnosis with cysto-urethrography and effect of ballooning therapy

Objective:To evaluate the application of retrograde cystography and voiding cysto urethrography in detection of posterior urethra valves in children as well as the dilatating effect of ballooning therapy.Methods: Sixteen children with posterior urethra valve, aged from 3 months to 4 years old with average of 2.5 years, underwent the managements mentioned above. In 16 cases, 13 were treated with dilatation by balloon catheter,3 subjected to surgical operation. All children suffered from dysuria and urinary stuffering after birth. Results:Cysto urethrograms demonstrated the filling defect caused by posterior urethra valve as well as the dilated posterior urethra and the features of a obstructive bladder. Vesico ureteric reflux was documented in 8 cases. Thirteen patients micturated normally one week following ballooning therapy and the curative effect had sustained during 1 to 3 years follow up. Conclusions:Retrograde cysto urethrography and voiding urethrography can well demonstrate the posterior urethra valve,the ballooning dilatation therapy is safe and effective,is a method of choice in treatment of the urethal obstruction by posterior urethra valve.
