内置式牵张成骨术在矫治火器伤性下颌骨畸形中的应用

摘    要:
目的:探讨内置式牵张成骨术矫治火器伤性下颌骨畸形临床应用的可行性。方法:为3例火器伤性下颌骨畸形患者延期(伤后2~3个月)行内置式牵张成骨术。术中常规截骨,制作骨转移盘,固定内置式骨牵张器,术后1周以每日1 mm的速度行骨牵引(8~22 mm)。固定期拍曲面断层片观察新骨生成及畸形矫治状况。结果:治疗按计划完成,无感染等并发症,骨牵引顺利完成;固定期3个月时,新生骨骨质、骨量良好,下颌骨畸形矫治效果满意,咬合关系良好。结论:在早期合理清创基础上,延期牵张成骨术可有效地矫治火器伤性下颌骨畸形,值得推广。

Abstract:
Objective: To summarize restorative results of mandibular deformities from gunshot wounds with the application of internal distraction osteogenesis. Methods: The present report consisted of 3 cases. Installation of internal distraetors started at 2-3 months after injuries. After a week of latency, the distraction started at a rate of 1 mndd, the distraction distance ranged 8~22 ram. The process of bone formation and effects of deformity correction was studied by general observation and panoramic radiography. Results: The patients and distractors were in good condition during the treatment. The bony transport discs were distracted as planned. In the consolidation period of 3 months, the new bone well formed within the distraction gaps, correction of mandibular deformity and restoration of occlusion were satisfied. Conclusion: On the basis of early debridement, the mandibular deformity could be corrected by the internal distraction osteogenesis effectively.
