超声实时应变成像技术在乳腺疾病性质鉴别诊断中的价值

摘    要:
目的探讨超声实时应变成像技术(量化超声弹性成像技术)诊断乳腺病变良、恶性的最佳诊断界点及其诊断价值。方法 分析经手术病理证实的83例不同病理类型良恶性乳腺结节的弹性成像检查结果,根据恶性肿瘤的特点,对病灶整体硬度比(病灶整体与正常腺体组织比值)与局部硬度比(病灶局部(1cm2)最高值-病灶局部最低值)进行量化分析;通过受试者工作特征(ROC)曲线了解该技术对乳腺疾病性质鉴别诊断的价值。结果病灶整体硬度比值对乳腺病灶良、恶性诊断ROC曲线下面积为0.926,其敏感性、特异性、阳性预测值、阴性预测值及准确性分别为:81.30%、92.20%、93.75%、92.15%和92.55%。病灶局部硬度比值对乳腺病灶良、恶性诊断的ROC曲线下面积为0.939,其敏感性、特异性、阳性预测值、阴性预测值及准确性分别为:90.60%、96.10%、96.87%、94.08%和95.18%。结论超声实时应变成像技术使用更为简便、客观、准确,有助于乳腺肿块的良、恶性鉴别。
