电离子导入5-氨基酮戊酸光动力疗法治疗皮肤肿瘤的实验研究

摘    要:目的评价局部电离子导入5-氨基酮戊酸(ALA)在光动力疗法(PDT)中的价值。方法①种植VX2肿瘤细胞制兔皮肤鳞癌模型;②治疗组于肿瘤局部通过直流电导入2%的5-氨基酮戊酸(ALA),持续30 m in,3 h后,进行光动力治疗(A组)。与此同时,设外敷10%ALA组(B组)、外敷2%ALA组(C组)、双蒸水导入组(D组),以及对肿瘤无任何干预措施组(E组)对照,观察、分析疗效。结果A,B,C,D组最大径平均减少值分别是0.376 cm,0.264 cm,0.06cm,0.07 cm。E组平均增大1.29 cm,A,B,C,D组与E组相比有显著性差异(P<0.05)。但其中以电导入加光动力治疗组疗效最佳,与B,C,D组相比有显著性差异(P<0.05)。A组ALA的用量是B组的1/5,疗效是C组的5倍多。结论电离子导入ALA光动力治疗可提高ALA-PDT的疗效。

Abstract:Objective To assess the value of iontophoretic topical delivery of 5-aminolevulinicacid(ALA) in the photodynamic therapy(PDT).Methods ①making rabbit skin squamous cell carcinoma model.isolating tumor tissues from the rabbit bearing VX2 tumor.Executing VX2 cell suspension with concentration of 1×10~7/ml,injecting 0.3 ml into the side back subcutaneous of 25 New Zealand rabbit.When the tumor grew to 1cm in its largest diameter,Grouping experiment as follow.②2% 5-aminolevulinicacid(ALA) was iontophoresised into the rabbit skin tumor 30 minutes,3h later,cured with photodynamic therapy(PDT)(group A),at the same time,compared with that of the two passive delivery groups,which were group 10% of ALA solution(group B)and 2% of ALA solution(group C).Also compared with the group that iontophoretic delivery of distilled water(group D),and group tumor without any treatment(group E).each group 5 rabbits.Then observed the growth curve and analyze the reduction of the largest diameter of each group.Results The mean reduction of the largest diameter of group A,B,C,D were 0.376cm,0.264cm,0.06cm,0.07cm respectively,but the mean diameter of the group E increased 1.29cm.Group A,B,C,D were all significantly effective compared with group E,P<0.05.But group A was the best one compared with group B,C,D.P<0.05.The ALA consumption of group A was one-fifth of that in group B,and its curative effect was more than 5 times better than group C.Conclusion It can significantly enhance the curative effect of ALA-PDT by iontophoretic delivery of ALA.
