螺内酯治疗慢性充血性心力衰竭疗效分析

目的观察小剂量醛固酮拮抗剂螺内酯治疗慢性充血性心力衰竭(CHF)的有效性和安全性.方法选择各种心血管疾病所致的CHF患者67例,随机分为观察组(A组)和对照组(B组),二组均给予常规综合疗法抗CHF治疗,A组联用螺内酯(20mg/d).治疗前和治疗后1个月、3个月、6个月分别进行心功能(NYHA)分级评估、超声心动图及生化检查,同时观察6min步行试验,并进行对照.结果治疗6个月A、B组心功能分级、无创心排血量指标、6min步行距离较治疗前均显著改善(P＜0.05),与B组比较,A组改善更显著(P＜0.05).血钾可见正常范围的升高,未发现严重高血钾及其他严重不良反应.结论应用螺内酯治疗CHF疗效肯定,安全性高.

EFFCT OF SPIRONOLATONE ON THE TREATMENT OF PATIENTS WITH CONGESTIVE HEART FAILURE

Objective To evaluate the clinical therapeutic efficacy and safety of spironolactone in patients with chronic congestive heart failure(CHF). Methods 67 CHF patients were divided into the treatment group(36 cases) and the control group(31 cases)randomly. Routine general therapy was given to the control group, the treatment group took spironolactone 20 mg per day besides routine general therapy. The heart function( by NYHA), echocardiography, biochemisty and six-minute walking test were performed before and after 1 month,3 months,6 months of the treatment. Results 6 months after treatment, NYH A class LVEF,SV,CO,CI and six-minute walking test improved significantly in both groups(P<0.05); but improved more significantly in group A than those in group B(P<0.05). Nobody got hyperpotassemia and other side effects. Conclusion Spironolactone is absolutely reliable and safe in the treatment of chronic congestive heart failure.
