MSCT诊断胆囊癌的临床价值

目的探讨MSCT在胆囊癌的影像学诊断中的价值。方法回顾性分析32例经手术或病理活检证实的胆囊癌患者的MSCT资料,所有病例均经MSCT平扫及三期动态增强扫描。结果胆囊癌MSCT直接征象表现为:肿块型16例、结节型6例、胆囊壁肥厚型10例,病灶均明显强化,伴有胆囊黏膜破坏、囊壁僵硬;间接征象有:合并胆囊结石14例、邻近侵犯12例、肝脏转移8例、淋巴结转移17例、胆道梗阻15例。29例术前诊断与手术病理结果一致。结论 MSCT平扫结合三期动态增强扫描可显示胆囊癌的特征性征象及常见的间接征象,对胆囊癌的诊断有重要作用。

Value of MSCT in diagnosing gallbladder carcinoma

Objective: To investigate the value of MSCT in diagnosing gallbladder carcinoma.Methods: MSCT findings of 32 patients with gallbladder carcinoma confirmed surgically and pathologically were analyzed retrospectively,and all cases were examined by CT plain scan and triphase dynamic enhanced scan.Results: Direct signs of gallbladder carcinoma on MSCT included: the type of mass in 16 cases,the type of intraluminal nodules in 6 cases,the type of thick wall in 10 cases,lesion with marked enhancement and destruction of gallbladder mucosa.Indirect signs included: concomitant gallbiadder stones(14),neighboring violations(12),liver metastasis(8),lymph node metastasis(17),biliary obstruction(15).The preoperative imaging diagnosis was consistent with surgical pathology results in 29 cases.Conclusion: The combination of CT plain scan and triphase dynamic enhanced scan can show the characteristic signs of gallbladder carcinoma and common indirect signs,which is important to diagnosis of gallbladder carcinoma.
