无症状性脑梗死56例临床分析

目的探讨无症状性脑梗死（SCI）的临床特征及其危险因素。方法对56例SCI患者的临床资料、影像学检查、危险因素进行分析。结果SCI好发于50～70岁,临床无神经系统定位体征,临床表现无特异性。病灶直径0．4～1．6cm。腔隙性脑梗死53例（94．64％）,大病灶性脑梗死3例（5．36％）;病灶位于内囊26例,基底节15例,放射冠区7例,丘脑3例,脑干2例。危险因素：高血压、动脉粥样硬化18例（32．14％）,糖尿病、高脂血症18例（32．14％）,冠心病、心房颤动5例（8．92％）。结论高血压、动脉粥样硬化、糖尿冠心病、高脂血症等均为SCI的危险因素。SCI无明确的神经系统定位体征,确诊有赖于影像学检查。

Clinical analysis of 56 patients with asymptomatic cerebral infraction

Objective To explore the clinical characteristics and risk factors of silent cerebral infarction （SCI）. Methods Clinical character, image characteristics and risk factors of 56 patients with SCI were analyzed. Results The most age of SCI was 50 - 70 years old. There was no typical clinical manifestation and focal neurological signs. The diamers of focus were 0.4 - 1.6cm. Fifty-three cases were lacunar infarction （94.64%）, 3 cases were macrofocus infarction （5.36%）. The focus of twenty - six cases were situated in capsula intema, 15 cases in basal ganglia, 7case in corona radiate , 3 cases in cerebral ganglia, 2 cases in brainstem. 18 cases were hypertension and artheroslerosis （32. 14% ）, 18 cases were diabetes and hyperlipemia （32.14%）, 5 cases were coronary heart disease and auricular fibrillation （8.92%）. Conclusion Hypertension, artherosclerosis, diabetic coronary heart disease and hyperlipemia are risk factor for SCI. There were no definite focal neurological signs in patients with SCI final diagnosis of SCI must depend on imageology detection.
