环甲膜穿刺术治疗呼吸道结核疗效分析

目的探讨环甲膜穿刺术治疗呼吸道结核的临床疗效。方法将138例呼吸道结核患者随机分为治疗组和对照组。治疗组采用环甲膜穿刺术加全身化疗，对照组只进行全身化疗。比较2组的治疗有效率。结果（1）喉结核治疗组及对照组治疗有效率分别为94．4％、41．7％（P〈0．01）；（2）肺结核合并糖尿病治疗组及对照组治疗有效率分别为62．5％、31．2％（P〈0．01）；（3）支气管结核并肺不张治疗组及对照组治疗有效率分别为66．7％、22．2％（P〈0．01）；（4）多耐药菌和空洞型肺结核治疗组及对照组痰菌阴转率分别为91．7％、62．5％（P〈0．01），空洞闭合率分别为58．3％、25．0％（P〈0．01）。结论环甲膜穿刺术治疗呼吸道结核疗效显著，可作为一种新的治疗方法推广应用。

Analysis on curative effect of thyrocricoid puncture on respiratory tuberculosis

Objective To analyze the curative effect of Respiratory tuberculosis treated by thyrovricoid puncture. Methods 138 Patients with Respiratory tuberculosis were divided into experiment and control group at random. Experiment group was treated by puncture through cricothyriod membraneand general antituberculotic, control group Being treated only by general antituberculotic . Results （ 1 ） The effective rate of experiment group and control group in throate tuberculosi cases was94.4% ,41.7% respectively（ P 〈0.01 ） ; （2） The effective rate of experiment group and control group in trberculoma accompanied by Diabetes mellitus cases was62.5% ,31.2% respectively（ P 〈 0.01 ） ; （ 3 ） The effective rates in endobronchial tuberculosis complicating ateleetasis and control group was 66.7% ,22.2% respectively（ P 〈 0. 01 ）; （4） The sputum negative conversion rates in multi drug - resistant tuberculosis and control group was 91.7% ,62.5% repectively （ P 〈 0.01 ） ; Cavity closure rates in multi drug -resistant tuberculosis and control group was 58.3 % ,25.0% respectively（P 〈 0. 01 ）. Conclusion Thyrovricoid puncture is a new effective method to treat in throate tuberculosi, trberculoma accompanied by Diabetes mellitus, multi drugresistant tuberculosis and in endobronchial tuberculosis.
