2型糖尿病患者脂肪细胞因子与下肢血管病变的相关性研究

摘    要:目的 探讨脂肪细胞因子与2型糖尿病患者下肢血管病变的关系.方法 严格选择符合2型糖尿病诊断标准三并排除1型糖尿病、糖尿病酮症酸中毒、肝肾心脏疾病以及其他内分泌疾病的患者100例,分为外周血管病变组(PVD) 18例和非外周血管病变组(NPVD) 82例.比较两组患者血清IR指数、瘦素、TNF-a、脂联素、空腹血糖、餐后2小时血糖等指标的差异.结果 两组2型糖尿病患者血清IR指数、瘦素、TNF-a、脂联素、空腹血糖、餐后2小时血糖在统计学上有明显差异(P<0.05),根据简单相关分析(Spear相关分析)和多元逐步回归分析表明脂联素与腰围、IR指数、BP呈明显负相关(P<0.05),瘦素与腰围、IR指数,TG 呈明显正相关(P<0.05),TNF-a与BP、IR指数、瘦素呈明显正相关(P<0.05),与脂联素呈明显负相关(P<0.05).结论 脂联素、瘦素、TNF-α在下肢血管病变发生、发展过程中起着重要的作用,ABI检测无创、客观、有效、费用低廉,对2型糖尿病患者下肢血管病变诊断具有重要意义.

Abstract:Objective To explore the relationship between adipocytokines and lower extremity vascular disease in patients with type 2 diabetes.Methods 00 cases which were strictly selected according to the type 2 diabetes diagnostic criteria and exclusion of type 1 diabetes,diabetic ketoacidosis,liver and kidney of heart disease and other endocrine diseases were divided into peripheral vascular disease group(18 cases) and non-peripheral vascular disease group(82 cases).ResultsThe serum IR index,leptin,TNF-a,adiponectin,fasting glucose,2-hour postprandial blood sugar in Two groups of patients with type 2 diabetes were statistically significant difference(P<0.05).According to simple correlation analysis(Spear correlation analysis) and multivariate stepwise regression analysis,Adiponectin was negatively correlated with waist circumference,IR index,BP(P<0.05),and Leptin was significantly related to waist circumference,IR index,TG(P<0.05).while TNF-a was positively correlated with BP,IR index,leptin(P<0.05),and Was negatively correlated with adiponectin(P<0.05).Conclusions Adiponectin,leptin,TNF-αwere played an important role in the development process of lower extremity vascular disease.The ABI detection is non-invasive,objective,effective and low-cost for type 2 diabetes patients with lower extremity vascular disease,and have important diagnostic significance.
