经皮改良三角肌联合肱肌分离入路锁定钢板治疗肱骨近端骨折60例临床分析

目的探究经皮改良三角肌联合肱肌分离入路锁定钢板治疗肱骨近端骨折的临床效果。方法选取2010年1月至2012年10月在中南大学湘雅医院接受治疗的60例肱骨近端骨折患者作为研究组,采用经皮改良三角肌联合肱肌分离入路锁定钢板治疗;另选取同期进行治疗的44例肱骨近端骨折患者作为对照组,采用传统手术治疗,比较两种手术方式的临床效果。结果研究组患者骨性愈合时间显著短于对照组(t=3.15,P=0.028);并发症发生率显著低于对照组(χ2=4.68,P<0.05);研究组患者肩功能评分优良率显著高于对照组(Z=8.32,P<0.05)。结论经皮改良三角肌联合肱肌分离入路锁定钢板疗法治疗肱骨近端骨折临床效果显著,值得临床推广应用。

Clinical Effect of Combined Percutaneous Brachial Deltoid Muscle Separation Approach with Locking Plate on 60 Patients with Proximal Humeral Fracture

Objective To investigate the effect of combined percutaneous brachial deltoid muscle separation approach with locking plate on proximal humeral fracture. Methods A total of 60 cases of patients with proximal fracture treated in Xiangya Hospital of Zhongnan University from Jan. 2010 to Oct. 2012 were selected as study group. Combined percutaneous brachial deltoid muscle separation approach with locking plate was used in the study group. Another 44 patients treated in Xiangya Hospital of Zhongnan University during the same period were included as the control group where conventional surgery was used. The effect of the two surgical methods was compared. Results Bone healing time in the study group was significantly shorter than the control group（ t = 3. 15,P = 0. 028）. The complication rate was significantly lower than the control group（ χ2= 4. 68,P〈0. 05）. Shoulder function scores were significantly higher than the control group（ Z = 8. 32,P〈0. 05）.Conclusion The therapy of combined percutaneous brachial deltoid muscle separation approach with locking plate has significant clinical effect on proximal humeral fracture,and is worthy of clinical application.
