Clinical study on the treatment of obesity with hyperuricemia by low purine diet combined with sodium bicarbonate and non bustard

Abstract:
Objective To explore the effects of low purine diet combined with sodium bicarbonate and febupropath on glucose and lipid metabolism,insulin resistance and renal function in obese patients with hyperuricemia.Methods One hundred and twenty four patients with obesity complicated with hyperuricemia admitted to Baoding First Central Hospital from January 20 to March 2019 were divided into observation group and control group according to random number table method.The control group was treated with low purine diet,and the observation group was treated with sodium bicarbonate and febupropathrin on the basis of the control group.The body mass index(BMI),blood pressure,glycolipid metabolism[fasting blood glucose(FPG),total cholesterol(TC),total triglycerides(TG)],insulin resistance[insulin(INS),homeostatic model insulin resistance index(HOMA-IR)],serum uric acid(UA),renal function and adverse reactions were compared between the two groups before treatment and 3 months after treatment.Results There was no significant difference in BMI between the two groups before and after treatment(P>0.05).while the systolic and diastolic blood pressures in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the levels of FPG,TC and TG in the two groups were lower than those before treatment(P<0.05),and the observation group was significantly lower than the control group(P<0.05).After treatment,INS and HOMA-IR in the observation group were significantly lower than those in the control group(P<0.05),while there was no significant difference between INS and HOMA-IR in the control group(P>0.05).After treatment,UA and Scr in both groups decreased compared with those before treatment,and those in the observation group were significantly lower than those in the control group(P<0.05).The incidence of acute gout in the observation group was significantly lower than that in the control group(6.45%vs.20.97%,P<0.05),while there was no significant difference in the incidence of elevated alanine aminotransferase and diarrhea between the two groups(3.23%vs.4.84%,6.45%vs.3.23%,P>0.05).Conclusion Low purine diet combined with sodium bicarbonate and febupropathol can effectively correct the disorder of glucose and lipid metabolism in obese patients with hyperuricemia,reduce insulin resistance,improve renal function,and have a high safety.
