辛开苦降法辨治恶性肠梗阻

恶性肠梗阻的病机为中焦气滞、痰浊、血瘀、热毒壅滞而致浊阴不降,腑气不通,病位在大肠,与脾、胃、肝、肾亦相关。针对邪实,以辛味药开壅滞之中焦,以苦味药降不通之腑气。辛味药主行善散,可助脾升清阳、运化精微以行气、活血、化湿;苦味药主泄善降,可助胃之降逆,通腑以祛郁结。辛苦同用,则脾升胃降,复司其职,清浊回归本位。

Treating Malignant Ileus by Method of Pungent Drugs for Dispersion and Bitter Drugs for Purgation

The pathogenesis of malignant ileus is stagnation of qi,phlegm,blood stasis and heat toxin in middle energizer,which leads to stagnation of turbidity and yin,and obstruction of fu organs and qi.The disease is located in the large intestine,related to the spleen,stomach,liver and kidney.In terms of pathogenic excess,pungent drugs are used for dispersion in the middle energizer,and bitter drugs for purgation.Pungent drugs for dispersion can promote the spleen to ascend clear yang,transport and transform essence to move qi,activate blood circulation and resolve dampness.Bitter drugs for purgation can help the stomach to direct counterflow downward,and dredge fu organs to remove stagnation.Pungent drugs and bitter drugs are used together to make ascend spleen qi and lower stomach qi,restore their functions and remove turbidity.
